Meet Our Team

Daniel LaPerriere, MD

Lead Physician

Dr. LaPerriere will be a guide for you in creating an integrative, individualized health plan that considers all aspects of your health, including genetic, environmental, and lifestyle factors. To ensure that you receive the custom-fit medical guidance that suits your health needs and your lifestyle, he combines his expertise in pharmaceuticals, innovative lab testing, supplements, botanicals, therapeutic diets, and mind-body techniques in treatment plans.

Prior to working for Foundation Health, Dr. Daniel LaPerriere served as Medical Director at Applewood Living Center, a nursing home in Longmont, as Medical Director of Boulder Manor Nursing Home in Boulder, and as a Family Physician at Yuma District Hospital.

He has been recognized for his outstanding work as a physician by a number of organizations. He received the 2005 Resident of the Year award from Rose Family Medicine Residency; the 2002 Arnold P. Gold Foundation Humanism Medicine Award; and the 2010 Partner in Quality Finalist of the Year, awarded by the Colorado Health Care Association in recognition of outstanding performance and dedication to long-term care residents.

Dr. LaPerriere graduated from the University of Medicine and Dentistry of New Jersey, Robert Wood Johnson Medical School in 2002. He performed his internship and residency at the University of Colorado Health Science Center, Rose Family Medicine Residency Program (a top-10 ranked residency program).

From 2005 to 2006, Dr. LaPerriere volunteered for 16 months as a physician in Chiri, Ethiopia. There he served as Medical Director of the Chiri Health Center. In addition to providing emergency, outpatient, inpatient, and OB care he provided medical education to Ethiopian nursing staff, managed programs in Tuberculosis and Leprosy, organized public health projects such as latrine building and nutrition programs for the Chiri community, and coordinated projects with local World Health Organization officials.

Returning from Ethiopia in 2006, Dr. LaPerriere worked with Docs Who Care, an organization that provides medical services to rural Colorado hospitals. Since then, Dr. LaPerriere has spent the majority of his time working simultaneously as a family physician and Chief of Medical Staff at Yuma District Hospital, and as Medical Director at Yuma Life Care Center Nursing Home. Through this experience he has seen and cared for patients in all spectrums of life, health, and disease; providing emergency room, inpatient, outpatient, nursing home, pediatric, and obstetric care to the local Yuma community.

Dr. LaPerriere is currently accepting new patients.

Shannon Ross, PA-C

Physicians Assistant

Shannon Ross moved to Colorado from California to complete her undergraduate education at the University of Colorado at Boulder. Upon graduating Summa Cum Laude with a major in Sociology, she attended Duke University, where she obtained her degree in Physician Assistant Studies and received a Masters degree in Health Sciences.

Shannon has spent the last ten years practicing in primary care. Her professional passions include chronic disease management and prevention, women’s health care and child development. She enjoys working with patients in all phases of life. She is also fluent in Spanish.

Shannon is currently accepting new Foundation Health patients.

Alison Merser Moore, PA-C

Physicians Assistant

Alison Merser Moore moved to Colorado from the coast of Maine in 1993. In 1994 Ms. Moore received her B.A. in Sociology and Women’s Studies from the University of Colorado at Boulder. She is a graduate of the University of Utah, where she obtained her Master’s Degree in Physician Assistant Studies and became the recipient of the Robert M. Alston Commitment to Excellence award in 2002.

Alison has experience in both Family and Internal Medicine. She has special interest in women’s health care, travel medicine and optimizing personal wellness.

She is board certified as a Physician Assistant and is a member of the American Academy of Physician Assistants, the Colorado Academy of Physician Assistants and the International Society of Travel Medicine.

Alison is not currently accepting Foundation Health patients.

Todd Dorfman, MD

Medical Director

Dr. Dorfman serves as the Medical Director of Foundation Health; the Director of Emergency Medical Services for the City of Boulder; the Chairman of the Boulder County Medical Directors Council; and the Associate Trauma Director at the Boulder Community Hospital. Dr. Dorfman is a full-time staff physician with the Boulder Community Hospital Department of Emergency Medicine and the Medical Director of American Medical Response. He is also an Associate Professor of Surgery at the University of Colorado’s School of Medicine. For the past decade, he has been the Medical Director and CEO of Cedalion Health, an executive health consulting group.

Dr. Dorfman develops Foundation Health best medical practices. Dr. Dorfman does not accept Foundation Health patients but does accept patients for non-primary care, consultative services at Cedalion Health.
