Foundation Health is the top primary care and family medicine experience in Boulder, CO. Dr. Jennifer M. Johnson, Dr. Eric Hernandez, Paul Young, Tiffany Bentsen, and Dr. Todd Dorfman are dedicated to giving you the best health care experience in Boulder. As a Foundation Health member,  you have access to a whole team of health care practitioners who have different health passions and expertise.


Jennifer Johnson, M.D.

Lead Physician

As a Foundation Health physician, Dr. Jennifer Johnson aims to provide a customized care experience and coordinated wellness approach to help inspire you to pursue optimal health.


Dr. Johnson is board certified by The American Board of Family Medicine and is a member of American Academy of Family Physicians. She is also an American Board of Integrative Holistic Medicine (ABIHM) Diplomate. She earned her BA in Human Biology from Stanford University and completed her Doctor of Medicine at The University of Colorado and her residency at St Anthony’s Hospital in Denver.


She is fluent in Spanish and has participated in medical research in Mexico. Dr. Johnson provides care to people of all ages, from wellness visits for children and adults; to managing chronic conditions; to treating hormone imbalance and providing gynecological care.

Paul Young PA-C

Paul Young, PA-C

Physicians Assistant

As a Foundation Health provider, Paul is excited to have the opportunity to serve you in your health goals. He joined the Foundation Health team because he values being able to spend time getting to know his patients–beyond just the symptoms. Looking at lifestyle factors and health concerns, Paul works alongside Dr. Johnson to create individualized health programs to address each patient’s acute care concerns and health goals with comprehensive preventive care.

Paul moved from New Mexico to Boulder to attend The University of Colorado at Boulder where he received his undergraduate degree in biology. He subsequently spent several years working as an EMT and Paramedic throughout Colorado and New Mexico. He went on to attend Emory University School of Medicine Physician Assistant Program earning a Master’s degree in Medical Science and graduating at the top of his class. He has also completed a certificate in global health through the Johns Hopkins School of Public Health and a fellowship in wilderness medicine through The Academy of Wilderness Medicine.

He has spent the last decade working in both primary care and emergency medicine throughout Colorado.

When not at work he spends most of his time with his two daughters and caring for his geriatric cats. Otherwise, as an avid climber and mountaineer you will find him enjoying the mountains of Colorado.

Paul is currently accepting new Foundation Health patients.


Todd Dorfman, M.D.

Medical Director

Dr. Dorfman serves as the Medical Director of Foundation Health; the Director of Emergency Medical Services for the City of Boulder; the Chairman of the Boulder County Medical Directors Council; and the Associate Trauma Director at the Boulder Community Hospital. Dr. Dorfman is a full-time staff physician with the Boulder Community Hospital Department of Emergency Medicine. He is also an Associate Professor of Surgery at the University of Colorado’s School of Medicine. For the past decade, he has been the Medical Director and CEO of Cedalion Health, an executive health consulting group.


Dr. Dorfman advises on Foundation Health best medical practices.


Dr. Dorfman does not accept Foundation Health patients but does accept patients for non-primary care, consultative services at Cedalion Health.

Eric Hernandez, M.D.

Lead Physician

As a Foundation Health provider, Dr. Hernandez is excited to have the opportunity to take time with you to listen and guide you in your health goals, rather than to dictate, a methodology that has become more the norm in past years in primary care. He joined the Foundation Health team because the Foundation Health model- with more time and better access- encourages a more thorough, thoughtful approach to primary care.


Dr. Hernandez was born in the Bay Area of San Francisco and moved to Littleton, Colorado when he was 2 weeks old. A passionate outdoorsman, Eric has always considered Colorado to be his home. He graduated from Colorado State University with a Bachelor’s Degree in Psychology and Master’s Degree in Anatomy & Neurobiology and attended medical school at the University of Michigan Medical School in Ann Arbor, Michigan and residency in Family Medicine at the University of Washington Family Medicine Spokane Residency Program. Both programs consistently rank in the top ten of family medicine residencies nationally.During and after residency he worked in rural Emergency Rooms, then transitioned to Urgent Care in metro Denver and concierge medicine throughout the Vail Valley. Continuing concierge care in Vail, he left Urgent Care to work in primary care as a physician in Family Medicine side-by-side with Dr. Johnson in Niwot.


During his time in Niwot, he was fortunate to care for many amazing people whom he grew very close to. However, as in most insurance-billing practices, appointment durations were cut to facilitate seeing more patients in less time. He realized his method of practicing medicine- a method that ignores the clock- was no longer viable in the traditional primary care model. He left that practice 4 years ago to exclusively pursue direct care.

He is Board certified in Family Medicine, a Member of the American Medical
Association, a Member of the Boulder County Medical Society, a member of
Colorado Medical Society, a member of the American Academy of Family
Physicians. He also serves as the Medical Coordinator for Helping Hands
Health Education in Boulder.

Tiffany Bentsen

Integrative Nutritionist

As Foundation Health’s Integrative Nutritionist, Tiffany believes that a thoughtful nutrition program can be the cornerstone of your health.

Tiffany has been a member of the Foundation Health team for over 3 years. She has a breadth of experience including being educated in Integrative Nutrition and having worked as a midwife for over 15 years. As midwife and childbirth educator, she recognized that nutritional education was key for a healthy pregnancy and healthy newborn. After moving to Colorado, she shifted her practice from prenatal nutrition to holistic nutrition, focusing on working to reverse and prevent chronic disease.

Tiffany is currently accepting existing Foundation Health patients into the FH Integrative Nutrition program.
